The TV mogul enjoyed a dinner at Sur in West Hollywood, then proceeded to leave the restaurant with Terri seated on his lap in the backseat of a car.
Simon and the Extra correspondent have remained on close terms even after they split in 2008.
And Terri certainly seemed comfortable as she perched on the X Factor judge's lap wearing a tiny black mini-dress, her dark hair in voluminous curls over her shoulders.
The pair appeared to be having a ball together, giggling as a blonde woman slid into the seat beside them.
Simon wore his usual uniform of jeans and a white T-shirt paired with black boots for his birthday bash.
His fellow former American Idol judge Randy Jackson also joined him for the celebrations.
The pair were seen affectionately patting each other as they said goodbye after dinner at the posh eatery, owned by Real Housewives' Lisa Vanderpump.
Earlier in the day, Simon and Terri were leaving Simon's Beverly Hills mansion and heading into the city's posh business district together.
After parking the car, Simon and Terri apparently went their separate ways but only for a short while.
He walked inside an office building while the former model breezed into a beauty salon to get her nails done.

Meanwhile, the media mogul had another surprise waiting for him when he arrived home on Monday to find that his Beverly Hills mansion had been 'toilet papered.'

It was a prank played by the girl band Fifth Harmony, who originally rose to fame on last year's X Factor USA and is composed of Ally Brooke, Camila Cabello, Dinah Jane Hansen, Lauren Jauregui and Normani Kordei.
